UAE pharmacies charge 0% VAT on medicines and medical equipment that appear on the official zero-rated list, and 5% on everything else they sell, including cosmetics, supplements and most baby products. Each product needs its own VAT code on the POS. Registration is mandatory above AED 375,000 of taxable sales, zero-rated sales included, and the VAT 201 is due by the 28th after each period.
- You own a community pharmacy, a pharmacy chain or a hospital outpatient pharmacy run as a separate business
- Your shelves mix prescription medicines with skincare, vitamins, baby care or personal care products
- Insurers pay part of your dispensing income and patients pay a co-pay
- Your combined medicine and retail sales are above AED 187,500 a year

Does a pharmacy need to register for VAT in the UAE?
Almost every pharmacy does, because zero-rated medicine sales still count as taxable supplies for the AED 375,000 mandatory threshold. A pharmacy that sells mostly 0% prescriptions cannot stay outside VAT on the argument that it collects little tax.
| Pharmacy situation | Registration answer | Why it matters |
|---|---|---|
| Medicines plus retail products over AED 375,000 in 12 months | Must register | Late registration is AED 10,000 plus the output VAT you should have charged |
| Total sales between AED 187,500 and AED 375,000 | May register voluntarily | Often worthwhile, since registration lets you recover VAT on rent, fit-out and 5% stock |
| Mostly zero-rated prescriptions | Still counted toward the threshold | You may be in a refund position every quarter |
| Several branches under one licence | One TRN for the legal entity | All branch sales are added together |
| Pharmacy run as a company | Corporate Tax registration as well | Every UAE company registers for Corporate Tax regardless of revenue |
An edge case: a clinic with an in-house dispensary may supply medicines as part of zero-rated healthcare services, which is a different analysis from retail dispensing. Our VAT for healthcare guide covers clinics, and our clinic bookkeeping guide covers the accounts side.
Which medicines are zero-rated for VAT in the UAE?
Medicines and medical equipment are zero-rated only when they appear on the official list issued under the VAT law, not simply because a pharmacist sells them. The list is item specific, so the question is always about the exact product, not the category on your shelf label.
How to check an item before you set its rate
- Match the product’s registered name and form against the current official list, not a supplier’s catalogue note.
- Keep a copy of the list version you relied on with the date you checked it.
- Recheck when a new product line arrives, when a brand changes formulation, or when the list is updated.
- Where a product is borderline, such as a medical device sold for home use, get the treatment confirmed before selling it at 0%.
Zero-rated is not exempt
Zero-rating keeps your right to recover input VAT on the rent, refrigeration, fit-out and stock that support those sales. Exempt sales would block that recovery. Our explainer on zero-rated vs exempt supplies shows why the difference is worth real money to a pharmacy.
Are cosmetics, vitamins and baby products taxed at 5% in a pharmacy?
Yes, unless a specific item is on the zero-rated list, products such as skincare, sunscreen, hair care, supplements, vitamins, baby toiletries and fragrance carry 5% VAT in a pharmacy exactly as they would in a supermarket. Being sold by a licensed pharmacist does not change the rate.
Typical rates across a pharmacy's range
| Product group | Usual VAT treatment | What to check |
|---|---|---|
| Prescription medicines on the official list | 0% | Exact item on the current list |
| Listed medical equipment (for example certain devices) | 0% | Device model and listing |
| Over-the-counter medicines not on the list | 5% | Do not assume OTC equals 0% |
| Vitamins and supplements | 5% | Unless the specific item is listed |
| Skincare, cosmetics, sunscreen, fragrance | 5% | Dermo-cosmetic brands are still 5% |
| Baby toiletries, nappies, feeding accessories | 5% | Check any listed infant medical products |
| Consultation or service fees charged separately | Depends on the service | Ask before invoicing |
Mapping every SKU to the right VAT code
The POS item master is where pharmacy VAT is won or lost. Set a VAT code per SKU, not per department, and lock the field so counter staff cannot override it. When a supplier adds a new barcode, the item should be blocked from sale until someone assigns its rate.
Run a monthly report of all 0% items sold and scan it for anything that looks like retail stock. Ten minutes on that report catches most errors before they reach the return.
Insurance co-pays on one prescription
When an insurer pays most of a prescription and the patient pays a co-pay, the VAT rate follows the item, not the payer. A zero-rated medicine is 0% on both the insurer claim and the co-pay; a 5% item approved on a claim carries 5% across both portions. Rejected claim lines that you later collect from the patient need the invoice and the claim record adjusted, so the sale is counted once.
How does a pharmacy file its VAT return?
You file the VAT 201 online through EmaraTax, after splitting the quarter’s sales into standard-rated and zero-rated totals and matching insurer claims to the POS. The steps below are the order we use for pharmacy clients.
Export POS sales by VAT code
Pull 0% and 5% sales separately for each branch, including returns and voids.
Reconcile insurer claims
Match submitted, approved and rejected claim values to the insurance portion recorded at the POS.
Check 0% sales for retail items
Review the zero-rated sales report for skincare, supplements or baby products coded wrongly.
Gather purchase invoices
Separate zero-rated medicine purchases from 5% stock, rent, utilities and equipment invoices showing your TRN.
Enter the VAT 201
Put standard-rated sales in the emirate boxes, zero-rated sales in the zero-rated box, and recoverable input VAT in the expenses box.
Decide on any refund
If input VAT is higher than output VAT, choose whether to request the refund or carry the credit forward.
Submit and pay by the 28th
Pay any net amount through EmaraTax on the same day to avoid the monthly late payment charge.
Which documents should a pharmacy keep for VAT?
A pharmacy should keep sales, purchase and rate evidence for at least 5 years, and be able to show why each 0% item was treated as zero-rated.
- POS Z-reports and sales exports split by VAT code and branch
- The dated copy of the zero-rated list you used and your SKU rate mapping
- Insurer claim submissions, remittance advices and rejection reports
- Simplified tax invoices or receipts for walk-in sales under AED 10,000
- Full tax invoices for sales to clinics, companies or other pharmacies
- Credit notes for returns and refunds
- Supplier invoices for medicines, retail stock, rent and equipment
- Stock write-off and expiry records
What VAT dates does a pharmacy owner need to track?
The date that matters most is the 28th of the month after each tax period, when both the return and payment are due. Your quarter-ends are set by the FTA and shown in EmaraTax.
| What | When | Pharmacy note |
|---|---|---|
| Invoice to a corporate or clinic customer | Within 14 days of supply | Walk-in patients can get a simplified invoice |
| VAT 201 for a period ending 30 September 2026 | 28 October 2026 | Include September insurer claims |
| VAT 201 for a period ending 31 October 2026 | 28 November 2026 | If your quarters are staggered |
| Corporate Tax return, December 2025 year end | 30 September 2026 | Separate from VAT, filed on EmaraTax |
| E-invoicing ASP appointment, revenue under AED 50M | 31 March 2027 | Go live 1 July 2027 |
| Reconsideration of an FTA decision | 40 business days | From the date you are notified |
What VAT penalties can a pharmacy face in 2026?
The penalty schedule is Cabinet Decision 129/2025, in force since 14 April 2026, and for pharmacies the incorrect return penalty is the one most often triggered, usually by POS rate errors.
| Violation | 2026 penalty | Typical pharmacy trigger |
|---|---|---|
| Registering late | AED 10,000 plus backdated output VAT | Assuming 0% medicine sales do not count |
| Filing the VAT 201 late | AED 1,000 first, AED 2,000 repeat within 24 months, per return | Waiting for insurer remittances |
| Paying late | 14% a year, calculated monthly | Cash tied up in unpaid insurance claims |
| Incorrect return | AED 500 first, AED 2,000 repeat | Cosmetics sold at 0% on the POS |
| Not issuing a tax invoice or credit note | AED 2,500 per case | Refund to a patient without a credit note |
| Records not kept | AED 10,000 for a first violation | No evidence of the rate mapping used |
| Voluntary disclosure | 1% a month before an audit notice; 15% plus 1% a month after | Correcting past POS mapping |
How it stacks: if a branch sold AED 180,000 of skincare at 0% in one quarter, the missing VAT is AED 9,000. Disclosed three months after the due date, that is AED 270 at 1% a month, and the FTA can also apply the AED 500 incorrect return penalty. Found in an audit instead, the 15% fixed charge alone is AED 1,350 before the monthly 1%.

7 VAT mistakes pharmacies make
Most pharmacy VAT errors start in the item master or at the insurance desk, and each of these produces either a wrong return or a missing document.
- One VAT rate for every product on the POS. Setting everything to 0% under-declares VAT on retail products; setting everything to 5% overcharges patients on listed medicines and misreports both boxes.
- Treating OTC medicines as automatically zero-rated. Only listed items are 0%, so an unlisted cough syrup or supplement sold at 0% creates a tax difference.
- Letting counter staff override the VAT code. Manual overrides leave no trail and are hard to defend in an FTA audit.
- Recording insurer payments on receipt instead of at dispensing. Sales end up in the wrong quarter and the return no longer ties to the POS.
- Refunds and returns with no credit note. Each missing credit note can cost AED 2,500.
- Not claiming input VAT because sales are mostly 0%. Zero-rated pharmacies keep full recovery; skipping it simply loses money.
- Assuming a refund position means nothing to check. Refund claims are reviewed, so wrong 0% coding surfaces exactly when you ask for money back.
How does a pharmacy stay VAT compliant every quarter?
A pharmacy stays compliant by controlling the item master, reconciling insurers monthly and reviewing the return before the 28th. This routine works for single outlets and chains.
- Monthly: review the 0% sales report for retail products coded wrongly
- Monthly: approve VAT codes for every new SKU before it goes on sale
- Monthly: reconcile insurer claims, rejections and co-pays to the POS
- Monthly: issue credit notes for every patient refund or product return
- Quarterly: tie POS sales, the ledger and bank receipts to the VAT 201
- Quarterly: file and pay by the 28th
- Every list update: recheck zero-rated items against the new version
- Annually: correct any past rate error through voluntary disclosure before the FTA writes to you
The UAE VAT return checklist has the full pre-filing list, and our guide to pharmacy bookkeeping covers batch stock and insurance receivables.
Missed a pharmacy VAT return or received an FTA notice?
Submit the overdue return and pay what is due today, since late payment is charged every month until the balance is cleared. Then fix the underlying rate errors and, if needed, challenge the penalty.
- File and pay the outstanding VAT 201. Use the best POS figures you have now and correct detail later.
- Disclose past POS rate errors voluntarily. Before an audit notice the charge is 1% a month of the tax difference; see our voluntary disclosure guide.
- Request reconsideration within 40 business days if a penalty is wrong, attaching your list evidence and SKU mapping. Our step by step FTA reconsideration request guide explains the process.
- Go to the Tax Disputes Resolution Committee if the FTA upholds its decision.
- Facing an audit? Our guide to responding to an FTA audit notice explains what to prepare.

Worked example: a Dubai pharmacy's quarter at 0% and 5%
This illustrative Dubai community pharmacy sells AED 600,000 in a quarter, with most of it from listed prescription medicines. The first part shows the correct return; the last lines show what one POS error costs.
| Line | AED | Working |
|---|---|---|
| Zero-rated listed medicines (insurer and co-pay) | 420,000 | 0% VAT |
| Standard-rated skincare, supplements, baby care | 180,000 | Net of VAT |
| Output VAT | 9,000 | 180,000 x 5% |
| Input VAT on 5% retail stock | 6,000 | 120,000 x 5% |
| Input VAT on rent and utilities | 1,500 | 30,000 x 5% |
| Medicine purchases | 0 | Bought at 0% |
| Total input VAT | 7,500 | 6,000 + 1,500 |
| Correct net VAT payable | 1,500 | 9,000 minus 7,500 |
| If all sales were coded 0%: return shows | 7,500 refund | 0 output minus 7,500 input |
| Tax difference | 9,000 | 1,500 payable vs 7,500 refund claimed |
| Voluntary disclosure 3 months after due date | 270 | 9,000 x 1% x 3 |
| Possible incorrect return penalty | 500 | First violation |
One wrong default on the POS turned a AED 1,500 payment into a AED 7,500 refund claim, which is exactly the kind of return the FTA looks at closely.
Is it cheaper for a pharmacy to file VAT in-house or outsource it?
For a single pharmacy with a clean POS, in-house filing is possible; once you run insurer claims across branches, outsourcing usually costs less than one incorrect return and the time spent fixing it.
| Approach | Cost | Owner or pharmacist time | Main risk | Best for |
|---|---|---|---|---|
| In-house (owner or pharmacist) | No fee, pharmacist hours diverted | High every quarter | Rate mapping never reviewed independently | Single outlet, few insurers |
| Freelance bookkeeper | Typical market range: modest monthly fee, varies by volume | Medium | Limited knowledge of the zero-rated list | Stable single outlet |
| Accounting firm (Paci) | Bookkeeping from AED 599/month; VAT filing on a fixed quote within 24 hours | Low | Reviewed by a qualified accountant with SKU rate checks | Chains, insurer-heavy pharmacies, refund positions |
Our VAT return filing service includes the POS rate review before each submission.
Questions pharmacy owners ask us about VAT
Is exempt the same as zero-rated for our medicines?
No. Listed medicines and medical equipment are zero-rated, which means 0% VAT with full input VAT recovery. Exempt supplies, such as residential rent after the first 3 years, bare land and local passenger transport, carry no VAT but block recovery of the related input VAT.
How do we work out the VAT inside a shelf price?
For a 5% item priced VAT-inclusive, the VAT is the price times 5/105, so an AED 105 moisturiser contains AED 5 of VAT. A listed medicine at 0% contains none, which is why each item needs its own VAT code rather than one rate for the basket. Our VAT calculator does the split.
Our input VAT is higher than our output VAT. Can we get a refund?
Yes, when recoverable input VAT exceeds output VAT the return shows a refundable amount, which you can request from the FTA or carry forward. Pharmacies with mostly zero-rated sales often land here. Our guide to the VAT refund process covers the request.
Can we file the pharmacy's VAT return online?
Yes. The VAT 201 is filed on EmaraTax, due by the 28th of the month after the period. A late return costs AED 1,000 the first time and AED 2,000 for a repeat within 24 months.
What is the simplest way to keep a pharmacy VAT compliant?
Map the right rate to every SKU, issue simplified tax invoices for retail sales under AED 10,000 and full tax invoices to business customers, and keep records for 5 years. A first record-keeping failure costs AED 10,000.
Frequently asked questions
Is there VAT on medicine in the UAE?+
Medicines on the official zero-rated list carry 0% VAT. Medicines and health products that are not on the list are standard-rated at 5%. The UAE healthcare VAT guide covers the wider sector.
Do pharmacies in Dubai charge VAT on vitamins?+
Usually yes. Vitamins and supplements are 5% unless the specific product is on the zero-rated list. The fact that a pharmacist sells them, or that they are health related, does not make them zero-rated.
How often does a pharmacy file a VAT return in the UAE?+
Most pharmacies file quarterly, by the 28th of the month after each quarter, on periods set by the FTA. See quarterly vs monthly VAT returns for how periods are assigned.
Does a pharmacy need to give a tax invoice for every sale?+
Walk-in sales to consumers under AED 10,000 can use a simplified tax invoice. Sales to clinics, companies and other VAT-registered buyers need a full tax invoice with both TRNs within 14 days. Refunds need a credit note.
Can a pharmacy chain register each branch separately for VAT?+
Not if the branches belong to one legal entity: that entity has one TRN and files one return, reporting sales by emirate. Separate companies register individually unless they apply to form a VAT group.
What is the penalty for charging the wrong VAT rate on medicines?+
The resulting incorrect return costs AED 500 the first time and AED 2,000 for a repeat, plus 1% a month on any tax difference if you correct it by voluntary disclosure before an audit notice. See the full UAE VAT penalties guide.
Does Corporate Tax also apply to pharmacies?+
Yes. A pharmacy company registers for Corporate Tax and files a return even if profits are low; 9% applies only to taxable income above AED 375,000. Our Corporate Tax guide for pharmacies explains stock and insurance claims.
